With the recent legalization of medical marijuana in Oklahoma, figuring out how to obtain a medical marijuana license can leave patients with a lot of questions. This is especially true for parents or legal guardians who have a child or teenager who qualifies for and would benefit from medicinal cannabis.
Thankfully, the process is straightforward, even if it does have a number of steps. This page will give you a step by step on how to acquire a medical marijuana card for a minor. What is a Medical Marijuana License for Minor Patients?
A minor patient license for medical marijuana in the state of Oklahoma permits an individual under the age of 18 to legally possess, grow and use medical marijuana and medicinal marijuana products in Oklahoma. It is acquired by obtaining two physician recommendations and a parent or legal guardian submitting an online form to the Oklahoma Medical Marijuana Authority (OMMA).
The card will consist of the patient’s name, date of birth, city and county of residence, license expiration date, type of license, a unique medical marijuana license number, and the patient’s photo. Medical marijuana cards for minors are valid for either two years or until the 18th birthday of the patient, whichever comes first.

A caregiver license may be issued to a parent/legal guardian.
How to Obtain a License:
Step 1: Get an Oklahoma Doctor's recommendation
First, the minor will need to attain recommendations for medical marijuana from two physicians. They will each need to fill out a different form and both of these forms will need to be included in the online application. The first will fill out the Minor Patient First Physician Recommendation Form, and the second will fill out the Minor Patient Second Recommendation Form.
Both forms must be dated within 30 days of each other, and the medical marijuana card will need to be applied for no later than 30 days after the issue date of the second form, otherwise they will be rendered invalid.

Step 2: Apply for an OMMA Patient License
An application must be filled out and submitted on the Oklahoma Medical Marijuana Authority (OMMA) website. Information needed on the application consists of:
For minor patient:
- Full name
- Date of birth
- Residence and mailing address
- Email address and telephone number (if applicable)
- Information from two recommending physicians
Required information about the parent or legal guardian:
- Full name
- Date of birth
- Residence and mailing address
- Email address and telephone number
- Signature dated within 30 days of application date
You will also need to submit valid, unexpired documentation for the minor patient, these include:
- Valid First Physician Recommendation Form
- Valid Second Physician Recommendation Form
- One proof of identity document (must be a color copy). Any of these items are acceptable;
- Oklahoma drivers license or identification card (front and back)
- United States passport or U.S. government-issued photo identification
- A certified copy of minor’s birth certificate (the seal/watermark must be visible on uploaded document)
- Tribal identification card approved by the Oklahoma Department of Public Safety for identification purposes. These include United States Bureau of Indian Affairs Identification Card or an Oklahoma tribal photo identification card: Muscogee (Creek) Nation of Oklahoma, Kiowa, Apache of Oklahoma, Comanche, Absentee Shawnee, Choctaw, Delaware, Cheyenne-Arapaho, Caddo Nation of Oklahoma issued on or after January 9, 2008.
- A clear, full-face, color digital photograph.
- Minor’s Medicare insurance card, Medicaid (SoonerCare) or enrolment documentation (if applicable)
Valid documentation is needed for the parent or legal guardian who is submitting the application as well. These include:
- Proof of Identity (must be a digital, color copy of one of the following)
- Oklahoma driver’s license (front and back)
- Oklahoma identification card (front and back)
- U.S. passport or other U.S. government-issued photo identification
- Tribal identification card approved by the Oklahoma Department of Public Safety for identification purposes. These include United States Bureau of Indian Affairs Identification Card or an Oklahoma tribal photo identification card: Muscogee (Creek) Nation of Oklahoma, Kiowa, Apache of Oklahoma, Comanche, Absentee Shawnee, Choctaw, Delaware, Cheyenne-Arapaho, Caddo Nation of Oklahoma issued on or after January 9, 2008.
- If an applicant has none of the above, a certified copy of applicant’s birth certificate will be accepted (the seal/watermark must be visible on uploaded document)
- Proof of Residency (must be a digital, color copy of one of the following)
- Oklahoma driver’s license (front and back)
- Oklahoma identification card (front and back)
- A utility bill for the calendar month preceding the date of application (cellular telephone and internet bills are not accepted)
- Oklahoma voter identification card
- Current rental agreement for residential property (located in the State of Oklahoma)
- Residential property deed to property in the State of Oklahoma
- A clear, color, full-face digital photograph
- If applicable, documentation establishing legal guardianship
Step 3: Payment
To submit the application, the cost is $100. Minor patients who provide valid proof of Medicaid (SoonerCare) or Medicare enrolment will have a cheaper fee of $20. This fee must be paid by either a Visa, Discover or MasterCard credit card or debit card. There is an additional credit card processing fee that occurs with the transaction. Payments are non-refundable and will not be refunded for applications not approved.
If the applicant does not have a credit or debit card, a pre-paid Visa or MasterCard may be used. These pre-paid cards may be purchased at a large number of retail stores, and the amount of the license and processing fees can be added at the retail store. The card can then be used for the online transaction.
Step 4: After you apply
When the application is submitted, it will be reviewed by the Oklahoma Medical Marijuana Authority (OMMA). If it is properly completed and meets all requirements, the applicant will receive an approval letter and the minor’s identification card within 14 days of when the application was submitted.
With incomplete applications (due to missing or unclear information or documents), the applicant will receive an email stating the reason(s) the application is incomplete and the applicant will have the opportunity to log in their account and make corrections. The submission date is the date the application is completed.
If the application is denied due to the applicant not meeting the requirements, the applicant will receive a denial letter stating the reason(s) for the denial within 14 days of when the application was submitted.
Once the Minor Patient has their Card
The medical marijuana card will be needed any time the minor patient and their guardian buys and transports their medical marijuana. When medical marijuana is purchased at a dispensary or when marijuana is taken to a processor to be put into a concentrate, the minor patient and their identified parent/legal guardian will need to show the medical marijuana card and another form of valid identification. Law enforcement may require to see identification cards from the minor patient and parent/legal guardian as well.
With more studies being conducted on the benefits of cannabis being done as it becomes legalized around the world, we are beginning to figure out just how much medical marijuana can help those with certain conditions or ailments.
